Packed Lunch Booking Form for Pupils in receipt of Free School Meals and Universal Free School Meals
To help staff quickly and efficiently cater for pupils who require a free packed lunch we are asking parents and carers of pupils, who are not attending school, to complete this booking form before 10am.
Guidance
- Please complete the booking form by including your full name and contact details.
- Please ensure that you include your child's dietary requirements in the 'dietary requirements' box.
- To ensure a meal is booked for your child, you must include your child's full name and class details in the 'additional information' box.
- You may also need to include the name of an 'appointed person' if you are unable to collect your child's packed lunch due to self isolation (this cannot be someone who lives in the same household).
- Press submit
- You will then receive a confirmation email.
This is intended as a temporary measure until food vouchers are issued by the Government.
Please note: We anticipate that this will be a short term solution of approximately 2- 3 days before the Government advised voucher scheme can be rolled out to parents and carers.
We strongly advise that parents and carers who do not need to bring their children with them, refrain from doing so.
Once all responses have been received and the number of lunches required are shared with the kitchen, parents and carers will receive a text message to confirm the time of collection for your child's free packed lunch and which area of the school to collect it from.
Upon collection of your child's packed lunch please adhere to social distancing guidelines and help us to ensure your safety and that of our staff, pupils and other families.
